Arden & GEM Commissioning Support Unit has an exciting opportunity for a CHC Clinical lead to work within the Northamptonshire CCG Continuing Healthcare team.

The post holder will be based at Francis Crick House, Northampton

The NHS Continuing Healthcare team in Northamptonshire works very closely with NHS Northamptonshire CCG.

An exciting opportunity has arisen for a registered nurse who has experience in a senior position to be part of this team covering the Northamptonshire area.

The role will be demanding requiring the successful candidate to be responsible for supporting and managing a team of nurses and the day-to-day work of the Continuing Healthcare Team (CHC). You will be expected to work closely with the Business Manager and administrative team to deliver a service on behalf of our patient population. Key collaboration is required with our partners in Health and Social Care, ensuring that the team works in line with National Guidance (NHS Framework for Continuing Healthcare).

The post holder should have experience of managing performance to ensure delivery of local and national standards including key KPI’s. The post holder should have excellent communication and clinical skills and knowledge and the ability to be able to work autonomously and as part of a team.

Having a positive approach and ability to support your team is fundamental, alongside having an attention to detail approach and the resilience to manage competing priorities.

The post holder must demonstrate a robust and comprehensive knowledge of the National Framework for NHS Continuing Healthcare and NHS Fund Care October 2018 (revised) and be able to demonstrate this on application.
